Dealing with a grandmother dealing with age related dementia

My grandmother is 81 years old. I have known for a few years now that her memory was getting worse. This week after she spent 6 days in the hospital I realized just how bad her dementia has gotten. I know sometimes I forget the day of the week and even the date.
